Prizes $600 Up To And Including $99,999.99
- Prizes $600 and above should be claimed by mailing to Michigan Lottery official website.
- Claims through the mail should fill out a claim form supplied by any Michigan Lottery office or available on the official website.
- You have to present two forms of identification as valid proof.
- Fill the back of your winning ticket, complete the Winner Claim Form mail it to the below mentioned Michigan Lottery P.O. Box address along with your signed original winning ticket, and copies of your valid government-issued photo ID and social security card.
Prizes $5,001 and Over
- Prizes of $100,000 and more, you will need to submit an online claim and visit Lottery headquarters, Hillsdale to claim your prize.
