For Prizes $1 - $599

All Arizona Lottery retailers are entitled to redeem prizes up to $100, and may also redeem the prizes up to $599.

For Prizes $600 And More

  • You must sign at the back of your lottery ticket
  • Fill the Winner Claim Form, available at the - lottery’s official website and at the Phoenix or Tucson Arizona Lottery office
  • You have to submit your signed lottery ticket, Winner Claim Form and ID

Submission In-Person

You have to deliver your completed Winner Claim Form, your signed winning lottery ticket and your government-issued ID to the Phoenix or Tucson Arizona Lottery office.

Submission By Mail

You have to send your completed Winner Claim Form, your signed winning lottery ticket and a copy of your government-issued ID (which includes your mailing address and signature) to the mailing address below. Keep a copy of all items, including the front and back of the ticket, for your records.
