HOW TO CLAIM WINNINGS?

Congratulations On Winning!

• Claim your prize before the deadline, i.e. within 180 days of the drawing for draw games and 180 days from the game end date for scratch games
• For EZ PLAY games, the tickets must be cashed within 180 days of purchase
• It is your responsibility that the information on the ticket is totally correct
• If you’re mailing the winning ticket, complete a claim form and attach a copy of your government-issued ID card and send it all together. Don’t forget to make a copy of the winning ticket (front and back) before mailing it.
• Sign the ticket before claiming because an unsigned ticket can be claimed by anyone in possession of the ticket
• Ensure that you possess the photocopy of the claim form AND your winning ticket (Both sides)
• Ensure that you include the original ticket (not the photocopy) with the completed claim form

Prizes Up To $599

• The retailers are entitled to redeem the winning tickets up to $599
• Make sure you have identification proofs for Photo ID verifying name, signature, age, and Social Security card

Prizes From $600 To $5000

• Contact a lottery retailer at your location. You will receive a Pay To Bearer Ticket from him
• After this, you have the option to either claim it from the agent retailer or fill the winner claim form, available on the official website and the retailer
• Or your retailer can also provide you with a list of cashing locations in your area where you may receive payment by presenting proper identification

Prizes Over $5000

• If you win more than $5,000, contact the nearest lottery regional office and make arrangements to file a claim in person
• Make sure you have identification proofs for Photo ID verifying name, signature, age, and Social Security card
